Transitional Care

This is recovery that feels like home, close to home. If you are recovery from surgery, illness or injury, our transitional care program cares for patients like family, providing skilled nursing close to home in a comfortable, family-friendly private room.

This service is for patients who need continued care and support through a recovery process before returning home – making sure a patient’s transition is a safe and smooth as possible from a clinical environment of a hospital to a patient’s home.

Through Bayside Community Hospital’s Transitional Care Program also referred to as the Medicare Swing Bed Program, provides the opportunity to receive personalized treatment plans with 24-hour care focused on patient’s needs and abilities from a skilled medical team. This step-down program helps strengthen patients and prepares them to resume daily life activities at home – making the transition from acute to follow-up care seamless.

Board-certified staff work together to give patients the highest quality of skilled care available. The teams include doctors, nurses, physical therapists, occupational therapists, respiratory  therapists, dieticians, pharmacists, social workers and activity support.
